Denial OS
How it works

Five frames. Five minutes.

Paste the EOB. Confirm the fields. Read the letter. Fax it. Mark the win. That's the whole product. Watch it run end-to-end.

Frame 01 — paste your EOB

Auto-advances every 4 seconds. Hover or click a frame to pause.

What's actually happening behind each frame.

01

Paste

Copy the EOB text from your payer portal or scanned image. We accept any format — clearinghouse export, payer PDF, plain paper. No PMS integration required to start.

0:00 — 0:30Average paste size: 1.2 KB.
02

Extract

Sonnet 4.6 reads the EOB token-by-token and returns 18 structured fields with per-field confidence scores. You see every field populate; you confirm or correct anything that looks off before moving on.

0:30 — 0:37Engine verified end-to-end on 2026-05-08 at 6.6s, $0.018 per extraction.
03

Letter

Opus 4.7 generates a 2–3 page formal appeal grounded in the payer playbook, the CARC code library, and your contracted rate from the TiC machine-readable file. The signature paragraph cites the rate by dollar amount with the source slug.

0:37 — 1:07Verified output: 779-word letter, attorney-grade voice, $0.183 per letter.
04

Fax

Phaxio transmits the PDF to the payer's appeal fax. We track every page, capture the delivery confirmation ID, and write it into the audit trail. Failed transmissions roll the appeal back so you can retry without re-typing.

1:07 — 1:38Average end-to-end fax delivery: 31 seconds at $0.07 per page.
05

Win

We remind you at 14 days, 7 days, and on the regulatory deadline. When the check hits, mark Won and enter the recovered amount. Your dashboard counter ticks up; the outcome data flows into the win-rate model. If they stall past the deadline, one click pre-fills a state insurance commissioner complaint.

14 — 30 daysCA commercial deadline: 30 business days. ~70% appeal win rate with the rate citation.
Denial OS
